Registration
  1. Registration should be filled out and turned in by the first day, August 18, 2008 for the year.All new and returning students are required to complete a registration form.


  2. You may register at any time during the year, provided space remains in that level or class.

  3. All returning SDA students are required to pay a $15.00 non-refundable registration fee. All new students are required to pay a $25.00 non-refundable registration fee.

  4. A family discount apply as follows: 10% off first additional child and 15% off second child enrolled and 20% off third child enrolled. All discounts apply with the highest percentage deducted from the lowest amounts of the tuitions paid. Family discounts will only apply to Quarters or Semesters paid in full and not to pro-rated or partial payment amounts. Parents will receive a $15.00 referral credit for each recommended student who registers with Scottsdale Dance Academy. Family and referral discounts will only apply if siblings or referred student enrolls for one full quarter or more. Referral discounts will be deducted from the next quarters tuition.

  5. Parents are encouraged to review all information given to students and to monitor the bulletin board in the lobby. Please inform us immediately of any conflicts with rehearsal and performance dates for either Winter or Spring Performances.


  6. You may fill out the registration form, attach your $15 (returning) or $25.00 (new) registration fee check and mail it to:

Payments
1) Payments and all fees should be made to Scottsdale School of Ballet or SSB. If mailing payment mail to the address listed above. We accept only cash or checks for payment of tuition and fees.

2) Payments at SDA are on a Quarterly or Semesterly basis and are as follows:

First Quarter: August 18th - October 18th
Quarterly Payment Due by August 22th
Semester Payment Due August 18th*

Second Quarter: October 20th - December 20th
Quarterly Payment Due by October 24th

Third Quarter: January 5th - March 13th
Quarterly Payment Due by January 9th
Semester Payment Due by January 5th*

+Fourth Quarter: March 16th - May 17th
Quarterly Payment Due by March 20th


*Semester payments are only accepted on or before the first day of the Semester. Otherwise Quarterly payments will apply.



Please note the above dates. You will not receive any notice or advance reminder that payment is due and all late fees will apply. So please keep a copy of these dates.

If you wish, you may make 2 payments per quarter by adding a $10.00 bookkeeping charge to each of the 2 payments. First payment is still due on the first day of the Quarter. Second payment is due 4 weeks after the first payment. Please make all payments to: Scottsdale School of Ballet. Remember, at this time we only accept cash and checks. If you are paying with cash, do not leave without a receipt.
3) Returned checks will result in a $25.00 charge to your account.
4) A "Late Fee" will be charged $10.00 per week. These charges will be automatically applied to your account and must be paid prior to performances. Any unpaid balance will result in the student not performing. If for some reason you are unable to make the payments by the due dates, please notify us prior to the due date, so we can make arrangements. Students whose accounts are not paid up to date will observe class until payment is received.
5) Tuition is non-refundable except in a case of serious injury or illness. Refund or credit will be on a pro-rated basis only for classes missed. Tuition will be refunded upon the date we receive the written request and will not be retroactive to any earlier date. (Doctor's note is required.)
6) Costume Fee: A non-refundable Costume Fee will be charged in a performance option. Costume fees are due with 2nd Quarter tuition and this year will cover both Winter and Spring Performances. All costumes will be kept, stored and maintained by SDA for use in future performances.
Pre-Primary - Level A $75.00
Level B & C $85.00
Level 1 - Level 3 $95.00
Level 4 - Advanced $110.00


7) "Production Rehearsal Fee" - This year there will not be a Theater Fee charged and no complimentary tickets will be issued. There will however be a "Production Rehearsal Fee" to cover additional in-studio costs for all Saturday's and the Rehearsal Week prior to the Performances. For all students participating in the Winter Performance this fee will be due the week of September 22nd 2008. For students participating in the Spring Performance payment is due with 3rd Quarter tuition January 5th 2009.
For all students participating in both Winter and Spring both payments are required on the aforementioned dates.
Pre-Primary - Level A $15.00 per payment
Level B & C $20.00 per payment
Level 1 - Level 3 $50.00 per payment
Level 4 - Advanced $75.00 per payment

Attendance and Dress Code

1) Students are required to attend and arrive 5 to 10 minutes prior to all classes for which they have registered. Students arriving late may be asked to observe class. Absences are allowed only for medical reasons, illness, injury or serious personal problems. If a student has several unexcused absences, or excessive tardiness this may compromise the student's participation in the "Winter Presentation" and/or "Spring Performance". Please notify SDA of all absences. All students must abide by the "Required Class Attire" for their level. Shoe size and fit must be approved by teacher or directors prior to student attending classes. Students not in the proper attire may be asked to observe class. All ladies hair should be pulled back in a bun with no bangs. No jewelry, gum, food or drinks (except water in a plastic bottle) will be allowed in the studios
2) For safety reasons for all students, if a student misses any one of the full run-throughs, technical, or dress rehearsal, the student may be asked not to perform.
3) Missed classes may be made up within the same quarter at the same or lower level classes.
4) Please keep in mind, for the betterment and safety of the student,it is not uncommon for certain students to repeat or remain in the same level for more than one school year.
5) Do not bring cash, jewelry, or other valuables to the studio. Scottsdale Dance Academy is not responsible for any lost or stolen articles.

SDA Viewing Days

For our students, viewing weeks are an integral part of learning to perform.We encourage parents to attend as often as you like during these weeks. Viewing days will be Monday thru Friday. Saturday's are not included in the Viewing Week. Because of space restrictions parents will only be allowed to view classes during the SDA Viewing Days. We welcome and encourage parents, family members and friends to come and see the progress of the students throughout the year. Please arrived prior to the class start time and turn off all pagers, cell phones. Siblings may attend but please, no babies or very young children during these classes. If you must leave during the class, wait until an exercise has finished. You may take photographs or videotape your child during these classes. Special arrangements to view classes outside of the Viewing Days may be made upon request of the directors. Viewing Days are:

February 23rd- 27th, 2009

Calendar

Upon registration your child will receive a calendar for the entire year. Included will be all days off, weeks off, vacation days, and holidays observed by SDA. Or you can also click here to access the CALENDAR
